Clinical application

Application of portable microinjection pump in the treatment of iron overload patients with blood diseases

Iron overload is mainly caused by abnormal iron metabolism in the body, which can cause heart failure, cirrhosis, diabetes and other diseases. Iron overload caused by blood diseases is mostly secondary to transfusion-related iron overload [1]. At present, iron removal therapy is the main therapeutic measure for iron overload in blood diseases, and portable micro-injection medicine is a device suitable for long-term administration. We apply portable microinjection dishes to the treatment of blood diseases
In the treatment of patients with iron overload and the implementation of corresponding nursing measures, satisfactory clinical efficacy has been obtained, as reported below.
The control group was treated with routine deferric therapy, that is, deferriamine mesylate (produced by Novartis) 1000 mg/d, dissolved in 0.9% sodium chloride injection, intravenous infusion for 6-8 h, continuous treatment for 3 months. At the same time, routine nursing is implemented, including strict implementation of various operating procedures, strict observation of vital signs, good oral care of patients, and patiently answer patients’ questions.
The experimental group was treated with portable microinjection dish with the same drug as the control group. The portable microinjection dish was produced by Shanghai Anjie Electronic Equipment Co., LTD., model AJ580. According to the doctor’s advice, different doses of deferriamine were dissolved in 10 ml water for injection or.9% sodium chloride injection. Subcutaneous injection was maintained for 8 ~ 12 h with portable micro subcutaneous injection herbs, once a day, 5 days a week, and the treatment time was 3 months. During the treatment for portable trace vegetables to take appropriate care: (1) care during venipentesis. The puncture site should be selected according to the specific conditions of the patient, with the outer thigh, upper arm and umbilical circumference as the first choice, but attention should be paid to avoiding red, swollen, ruptured and itchy skin. The patient is advised to do less bending during the treatment to prevent the puncture from causing friction to the subcutaneous tissue and resulting in needle folding, pain, subcutaneous bleeding, etc. (2) Set parameters of micro-injection dish. Follow the doctor’s advice to select the flow rate of micro-injection vegetables, after starting the injection switch, the indicator can be seen to flash, and the infusion speed is set to uniform speed; Adjust the appropriate length according to the specific situation of the patient, and it is appropriate to inject a small amount of vegetables across the chest or waist of the patient, but it should be noted that it does not affect the toilet, eating, walking and other actions. (3) Condition observation. Observe the effect of deiron treatment, and ask the patient to review the serum ferritin regularly; Explain to the patient that the red or reddish-brown urine during treatment is caused by the combination of desferriamine with trivalent iron ions in the body to form feroxamine B, and gain the patient’s understanding; Make good observation of adverse reactions, explain the reasons to the patient in time and propose corresponding treatment measures to eliminate the tension of the patient. (4) Dietary guidance. Chronic aplastic anemia and myelodysplastic syndrome were the main cases in this group, so increasing iron in the diet has no direct therapeutic effect on the improvement of anemia. Therefore, patients should be instructed to eat less or no iron-rich foods, such as kelp, fungus, pig liver, bananas, etc., and avoid the use of iron cookware, while taking vitamin C to increase the excretion of iron in the urine.
3 Discussion
Iron overload can induce a series of biochemical reactions in the body, resulting in the damage of the body’s antioxidant system, resulting in the damage of cell membrane and DNA. Some special types of blood diseases with severe anemia need to rely on long-term blood transfusion to improve the anemic state, but long-term dependence on red blood cell infusion can easily cause iron overload and lead to multiple organ complications such as liver and heart. Iron removal therapy is the main measure at present, and the effect of using portable micro-injection vegetables to remove iron treatment has been fully affirmed by the clinic. The results of this study showed that the experimental group had higher comfort and less adverse reactions during treatment, and the improvement of hemoglobin and serum ferritin was more obvious after treatment. In the process of continuous drug infusion, patients do not need to stay in bed for a long time, daily life can be maintained under the condition of physical conditions, physical and psychological comfort are better, and coupled with relevant nursing measures, effectively reduce the incidence of complications, so that the treatment effect is significantly improved, and the clinical efficacy is satisfactory.
